A patient medication profile is a comprehensive summary of all regular medicines taken by a patient. The patient medication profiles that are currently available may differ in format and the level and type of information provided.

The provision of a patient medication profile service in community pharmacies will provide support to eligible patients to enable them to manage their medications confidently.  Furthermore, the service will assist with communication between the patient and other health professionals, especially if the patient is admitted to hospital or a nursing home.
